Clinical Pharmacy Specialist

Salary: $83.92 – $134.17 per hour
Providence Medical Group is seeking a Clinical Pharmacy Specialist to provide advanced ambulatory clinical pharmacy services in collaboration with physicians and interdisciplinary care teams. This role focuses on chronic disease management, medication therapy management (MTM), and population health initiatives.
Key Responsibilities

Provide collaborative drug therapy management

Optimize medication regimens for chronic diseases

Deliver patient counseling and medication education

Utilize registries for population health management

Support quality improvement initiatives

Precept PGY-1 and PGY-2 pharmacy residents

Provide formal and informal clinical education
Required Qualifications

Bachelor’s Degree in Pharmacy OR PharmD

PGY-1 and PGY-2 Pharmacy Residency OR equivalent experience

Oregon Pharmacist License (required upon hire)

Board of Pharmacy Specialties (BPS) Certification within 2 years

2+ years Senior Ambulatory Clinical Pharmacist experience
OR 5 years pharmacy practice experience with 2 years in relevant clinical setting
Preferred Qualifications

Board-certified (BCPS, BCACP, or similar)

Pharmacy Preceptor License (if applicable)

Experience in ambulatory care or chronic disease management
